The Union City Dental Aesthetics (UCDA) Dental Membership Plan is designed to provide affordable, accessible and high-quality dental care.

 

With your UCDA DENTAL MEMBERSHIP PLAN, there are:

* The Dual Plan is for Parent/Child or Husband/Wife only.

** The Family Plan includes family members and children who are enrolled full-time in college until the age of 18.

 
 
 

Our Dental Membership Covered Procedures (at 100%):

  • (new patient, initial visit)

  • (problem focused - 1 per year)

  • First film

  • Each additional film

  • 2 per year

  • 2 per year

  • 1 per year

    Child: Age 18 - 2 per year

  • 1 every 5 years

  • 2 per year, no age limit

  • 1 set per year

 

All other basic and major dental procedures done in our office will be given a 20% discount.

PROGRAM ECLUSIONS AND LIMITATIONS

This program is a membership plan, NOT a dental insurance plan, it cannot be used:

  • In conjunction with another dental plan.

  • For services for injuries covered under workman’s compensation.

  • For treatment, which, in sole opinion of the treating dentist or doctor, lies outside the realm of their capability.

  • For referrals to specialists.

  • For hospitalizations or hospital charges of any kind. For costs of dental care which is covered under automobile/medical insurance.

  • In conjunction with coupons or cash discounts.

When Care Credit is used in conjunction with this plan, your member discount is 5% of all procedures, except for those covered under 100%.

This dental membership plan is NOT an insurance plan that can be used with any other dental office. It cannot be used in conjunction with another dental office. It cannot be used in conjunction with another dental plan.

  • No Credit or Refund and is Non-Transferable.

  • No refunds or premiums will be issued at any time if participant decides not to utilize this dental membership plan.

  • Plan is valid for 12 months from date of sign-up.

  • Patient’s portion of payment is due at time of service.
